Implementation and analysis of Pharmaceutical Care based on the communication between Community Pharmacists and Primary Care Physicians through a two-way online platform using the MEDAFAR communication language

Objective: To test a direct communication line between physicians and pharmacists to exchange information and manage drug-related problems and negative outcomes associated with medication identified by community pharmacists.
Methods: Cross-sectional opportunities study, with voluntary participation, performed in Aspe, Alicante. Every pharmacy owner from the community pharmacies and every primary care physician in the municipality participated.
Results: Drug-related problems appeared in 34,6% of the users, with 4,0 on average per user. The level of chronicity in the study population included 190 participants in chronicity level 0, 1.746 in level 1, 1.041 in level 2 and 592 in level 3. 14.233 derivations were performed, out of which 13.317 (93,6%) were out of Necessity, 684 (4,8%) involved Effectivity, 194 (1,4%) involved Information and 38 (0,3%) involved Security, with 93,0% being accepted by primary care physicians, 5,6% rejected, while 1,4% received no reply. The average yearly savings per user was quantified at 35,9€ and at 12,4€ per target user.
Conclusions: The use of a platform for direct communication between community pharmacists and primary care physicians using the MEDAFAR language enables the practice of a measurable Pharmaceutical Care, which favours the collaboration in the detection and solution of drug-related problems, with complex chronic patients being those who benefit the most. It improves efficacy, efficiency and security of healthcare, reducing the load on health centres and optimizing resources in the healthcare system.
