Abstract

Prescription errors are among the most significant medication-related problems and a leading cause of negative medication-related outcomes. Community pharmacies play a key role in their early detection

We present the case of a patient referred from ophthalmology with a prescription for oral prednisone, dexamethasone eye drops, and atropine in injectable ampoules. The suspicion of an error regarding the dosage form prompted a pharmaceutical intervention coordinated with primary care and primary care pharmacy services. After 48 hours of clinical management, the prescription error was confirmed and replaced with atropine eye drops, allowing the treatment to begin.
