Community Pharmacy-Primary Care Communication Models in Spain for Integrated Healthcare Delivery

Abstract

Introduction: Community Pharmacies (CPs) are healthcare establishments of public interest whose coordination and collaboration with Primary Care (PC) remains limited in Spain, hindering the development of an integrated healthcare practice. The objective of this study is to analyze and evaluate the existing communication models between CPs and Health Centers (HCs) in Spain, which aim to establish effective communication between healthcare professionals.

Method: The Service-Learning methodology was used in the design and planning of the study. A bibliographic search was conducted in PubMed and direct contact was made with the Pharmacists Associations to collect and analyze documents, protocols, and experiences of interprofessional communication models already implemented in Spain.

Results: Communication between CP and HC is still an emerging field, with scarce published information and a still very limited degree of implementation in Spain. Structured communication models were identified in Asturias, Navarra, the Basque Country, and Zaragoza. The consolidated models implement secure digital channels for incident management, with high traceability and defined resolution times.

Conclusions: It is essential that communication models between CP and PC advance in the implementation of bidirectional effective channels that extend to other levels of care. This action must be accompanied by a structural framework that fosters a culture of trust and provides the institutional support necessary to recognize and sustain CP care activities, promoting their integration into PC.
