Clinical case: pharmaceutical interventions in a complex patient with alterations in the urea cycle

A patient with no previous diagnosis of carbamoyl phosphate synthetase 1 (CPS1) enzyme deficiency is admitted with progressive deterioration of consciousness and requires admission to the intensive care unit (ICU). From a pharmaceutical point of view, this is a complex patient, in whom the treatment of the metabolic disorder is approached with specific recommendations on the special requirements that enteral (EN) and parenteral (PN) nutrition must contain, on the one hand, and the development of metabolic acidosis and alteration of biochemical parameters, on the other. The patient required numerous interventions by the pharmacist, and the role of the pharmacy service was fundamental to the patient’s recovery.
