Assessment of treatment adherence in patients receiving acenocoumarol in a telecare program

Introduction: Adherence to treatment with oral anticoagulants is essential to ensure the effectiveness and safety of the therapy. This study evaluates therapeutic adherence in patients treated with acenocoumarol using the Morisky-Green questionnaire, the patient's experience with the IEXPAC instrument and the safety in the use of this drug.
Method: A study was carried out with patients over 18 years of age with cardiovascular disease treated with acenocoumarol, included in a programmed delivery program and telepharmacy follow-up. A retrospective review of medical records was performed to evaluate adherence to treatment and hospitalizations in 2023 and 2024. Prospectively, structured telephone interviews were conducted to evaluate adherence and satisfaction with the care received.
Results: Ten patients prescribed acenocoumarol were included (7 men and 3 women with a mean age of 57 years, from a registry of 179 patients). 81% discontinued the medication quarterly and all had INR controls; one patient was hospitalized in 2024 for treatment-associated hematuria. Regarding the results of the telephone interviews, 87.5% identified their cardiologist and showed adherence, 63.5% did not recognize the warning signs of treatment, 87.5% expressed satisfaction with the care received and only 70% received information on diet and medications. This last aspect highlights the need to strengthen health education, which is why a workshop for patients was organized and an educational brochure was developed.
Conclusions: High adherence was identified although there is a lack of information on the safety of the use of oral anticoagulants. Satisfaction with care was high.
