Study of the effectiveness of pharmaceutical intervention in improving therapeutic adherence in patients with asthma and COPD

Introduction: Drugs administered by inhalation are recommended for the treatment of respiratory diseases, but they present difficulties in their management and therapeutic adherence with these drugs is less than 50%, which leads us to study the effectiveness of the intervention of the community pharmacist to improve the process of using these devices, increase therapeutic adherence and control of these pathologies.
Method: A non-controlled, non-randomized prospective study. The Therapeutic Adherence Service assessed adherence with the Morisky-Green test, asthma control with the ACT questionnaire, and the measurement of Peak Expiratory Flow (PEF), COPD control with the CAT questionnaire, and perceived quality of life with the Visual Analog Scale (VAS), in two visits.
Results: Of the total patients included in the study (n=80), 62.5% had asthma and 37.5% had COPD. Among asthma patients, adherence increased from 30.0% to 68.0% with a 1.9-point improvement (SD: 3.2) in the ACT test, and 80.0% either improved or maintained their quality of life. Among COPD patients, adherence increased from 16.7% to 53.3%, with a -2.4-point improvement (SD: 6.0) in the CAT test, and 83.3% either improved or maintained their quality of life.
Conclusions: The provision of the Therapeutic Adherence Service increased the number of adherent patients by 35.0% and showed a positive impact on the clinical control of both conditions and on quality of life.
